Creating connections

The Creating Connections program provides short-term, case managed support to help young people aged 18 to 24 who are experiencing or at risk of homelessness to find appropriate housing.

Wesley, in partnership with UnitingCare Harrison, Family Access Network, SalvoCare Eastern and Anchor, delivers Youth Focused Housing Placement as part of Creating Connections.

We provide brief intervention crisis support, which includes assessment, outreach, co-case management, secondary consultation to the youth homelessness sector and referral and support to access youth specific crisis accommodation options. Our team also focuses on early intervention so that young people do not leave the family home.

We assist each young person at the refuge to address and resolve the issues that may have contributed to their current situation and move forward, out of homelessness. In addition to accommodation, the refuge also offers:

  • case management
  • short term counselling
  • outreach support
  • life skills development
  • education, employment and training support
  • advocacy
