![]() Wesley provides support to young people in need, who do not require protective care, through case management and outreach support.
Who does the program support?Wesley's adolescent support program supports:> Young people up between the age of 12 and 17 > Young people who are deferred from the Department of Human Services after assessment that they do not require protective intervention but some ongoing support is required > Young people referred to Wesley in need of support in regards to education, health or behavioural management > Families with young people who require assistance in ensuring the family unit stays together in a safe environment How does the program work?Wesley's adolescent support program works with young people living in many different situations. They can be living at home with their family, in accommodation they have found themselves, in transitional housing or living in the out of home care sector.Wesley will receive a referral from either someone within the community, the Department of Human Services or an external agency in relation to a young person who may require additional support. The young person will then sit down with a case manager to determine their goals for the future. Wesley commences working with young people once the young person?s goals and the outcome of the support are identified. Often, when working with young people who are already in out of home care, the support will be in the role of child protection.
