Wesley Disability Community Services Intake & Assessment

Our Intake & Assessment team is available to assist anyone who is interested in participating in the WDCS programs. We can be your first point of contact for all programs and will work with you to:
- Determine the supports that you are looking for and how best WDCS can provide these
- Complete referrals to specific WDCS programs, determine suitability and assist with transition where necessary
- Provide information on external services that may be more appropriate

Wesley Adult Training and Support Service
At Wesley ATSS, we tailor our program delivery to address each individuals' needs as identified through discussions with the individual, their family and carers.
The service accesses a wide range of community facilities and venues in the delivery of the program and activities. In addition the facilities at the centre have been developed to meet the needs of all people with disability.
Some of these specialised facilities include:
- Sensory stimulation room
- Well equipped Art Room
- Vegetable garden
- Wide open spaces
The majority of programs and activities are provided in small groups. Should a person require more individualised service then this can be negotiated between the client/carer, Wesley and DHS.

Wesley Shared Supported Holidays
Organised supported holidays for people with a disability that focus on matching the right individual for holidays. This ensures client choice and positive outcomes. We provide a range of holidays for different client groups:
Disability Holidays
These holidays are open to people who are intellectually and/or physically disabled and live in the state of Victoria.
ABI Holidays
Run in conjunction with the external partners, these holidays are open to anyone with Acquired Brain Injury (ABI). Specialised staff with experience in ABI provide support to all participants. Holiday programs are flexible and the schedule is designed by participants.

Wesley Community Access Planner
A therapy service assisting people with Acquired Brain Injury / disabilities to participate in their chosen recreation activities. This is a fee for service program that supports individuals to successfully link into chosen community recreation options.

Wesley Connect South
A community based program for people with Acquired Brain Injury (ABI).
Wesley Connect South works in partnership with community organisations, government and non-government agencies to establish program initiatives which enhance the quality of service delivery for people with ABI within the Southern Metropolitan region of Melbourne.
The program's objective is to improve the quality of life for people with ABI through services which are responsive to individual needs, wishes and abilities.
Wesley Connect South also operates training workshops for Attendant Care Agencies and other services which identify best practice methods for supporting a client with ABI to participate in community activities.
Wesley Connect South Cricket
On Tuesday nights, Steve (not his real name), a former panel beater with Acquired Brain Injury from a traffic accident, gets together with his mates for cricket training. The cricket group, set up by Wesley and Cricket Victoria 2 years ago, provides people like Steve with the opportunity to socialise in a supportive, recreational environment. Steve has improved his sporting skills and developed the confidence to explore other social options.

- Professional support to help integrate individuals into community activities of their choice
- Consultation/educational workshops for Attendant Care Agencies and other services focussed on how to assist people with ABI to participate in meaningful community activities
- Partnerships with community organisations to operate rehabilitation and recreational programs for people with ABI

Referral process
- Initial enquiry by client, therapist, case worker/family
- Information package identifying available services (including referral form)
- Written approval for assessment sent to Wesley Connect South prior to commencement
- Meeting with client, therapist or relevant others to complete assessment, including recommendations
- Written assessment sent to client, case worker, funding body
Costs
Fee for service applies for:
- Individual assessments for adults and children
- Attendant Care Agency and other services training

Wesley Connecting Communities programs enhance the quality of life for people with high support needs by offering a range of flexible, skill-based community activities that are responsive to individual needs and capabilities.

Costs
Activities incur a core hourly rate, which covers networking, organisation, planning of programs and staff support.
Each participant will incur a term fee.
Transport costs may apply.
Participants are required to cover any additional costs incurred through their participation in community programs.
Activities
Participants choose their own activities from a wide range of rehabilitation, recreation, educational and vocational programs, including:
- Numeracy and literacy programs in community based facilities
- Volunteer work
- Swimming classes
- Ceramic and creative art classes
- Computer courses
- Horse and buggy riding with Riding for the Disabled
- Wide range of activities offered by Neighbourhood Houses

Wesley Kids Under Kanvas
Wesley Kids Under Kanvas provides young people (aged 8-25), with a disability, the opportunity to participate in recreation and camping activities which develop new skills and self-confidence.
Specially selected volunteer leaders are trained to provide the safest environment and supportive encouragement for the development of group skills and self-confidence for young people. Volunteer leaders can be involved in coordinating and leading a camp or day activity, office administration, fundraising and catering among other roles.
