Wesley has 18 supported accommodation houses for adults with physical and intellectual disabilities in Eastern and Southern Melbourne.
Our purpose built residences are located in suburban streets with good access to local facilities such as shopping centres, recreational and leisure sites.
Who does the program support?
> Adults with physical and intellectual disabilities, who require 24 hour residential care, who would like to live in residential services in community settings with appropriate support.
How does the program work?
Wesley provides a holistic support service using person-centred approaches with attention to individual needs, wishes and dreams. Each resident works with family, friends, advocates and support staff to develop a personal support plan that outlines what the person themselves wishes to do.
Each house has a regular rostered staff complement and is led by a house Team Leader. Each resident has a key worker to work closely with them in developing their plan and to ensure that their particular interests and needs are being met. We encourage all residents to participate in the life of their local community through their person-centred plan. Our specially trained staff support residents to achieve their goals.
All residents attend a day placement during the week and access the community on a regular basis. Staff support is provided at all times residents are at home, with support available for holiday times, medical appointments and leisure activities.
