The planning may incorporate accommodation options, personal care, recreation, supported employment, day service options and more. The aim of the planning is to explore the person’s formal and informal networks and link the person with appropriate community or specialist support where necessary.

Who does the program support?

Planning and support programs are provided to:

- People with disabilities (intellectual, neurological, physical, acquired brain injury and degenerative conditions), who receive an Individualised Support Package (ISP) through the Department of Human Services
- Students with disabilities who are about to leave school and have a Futures for Young Adults (FFYA) or ISP package
- People who have an acquired brain injury referred by TAC

Referrals for this program must come through either the Department of Human Services in your metropolitan region or through TAC.

How does the program work?

Wesley staff meet with the person with the disability and their support network, and in relevant cases, their school, therapist and support coordinator, and explores with them their goals for the future, and what support they need to achieve those goals.

Wesley submits a proposal on behalf of the person for funding through either the Department of Human Services or TAC that will assist the person to achieve their goals.

Wesley may then assist to implement these supports with the person and in some cases reviews the plan with the person when required or on a three year cycle. Wesley is involved for a short amount of time only while the plan is being developed.
