 Wesley has several aged care accommodation options in Melbourne's north that provide differing levels of care and assistance to older people, including:
> Independent Living Units > Community Care > Residential Care Our fully accredited aged care facilities offer a pleasant environment in which the independence of residents is respected and supported. Our Independent Living Units in Coburg offer a safe environment in a communal setting. Some residents have a CACPS package that provides support. Who does the program support?To apply for our facilities, we accept referrals from: > Aged Care Assessment Team > Social Workers > Doctors > Local council > Hospitals > Families All clients require a valid aged care assessment and permanent clients require a completed assets and assessment test for permanent residential care. How does the program work?Our Independent Living Units in Coburg have a community feel while allowing people to remain independent. For people with a low care Aged Care Assessment, a Community Aged Care Packages (CACPS) may be offered to people in need of support with cleaning, shopping, bathing, medication, assistance with appointments, laundry, food preparation, meals and case management.Our units provide: > Heating and cooling > Self-contained units with ensuite bathroom and kitchenette > A safe, secure environment in which residents' independence will be respected and supported > 24 hour personal alarm system to summon help in an emergency > Shared laundry, dining, recreational and garden areas Our Community Care, Gilgunya Village, is an accredited aged care facility that provides residential support to people in beautiful surrounds. People living here enjoy a larger than usual bedroom and share meals and lounge areas with five or six people. An active lifestyle program allows residents to enjoy a range of activities either individually or with others. There are also Independent Living Units on the Gilgunya site for people who wish to live without additional support. Spending time at Gilgunya through the respite program provides a good introduction to living at the village. Emergency respite is also available. Referrals can be made through CarerLinks North, 1800 059 059. The community-based Residential Aged Care homes are a unique service located around the City of Moreland. This service is accredited under the Federal Government Aged Care Act. We provide support to people who have a long history of disadvantage through community based shared houses. Residents live in group homes, with single bedrooms and shared living areas. Our 'ageing in place' capacity means that as residents' care needs increase, they have the option to stay in the same place.
